Upon referral to the University Sleep Disorders Center or an appointment is given to a patient in sleep clinic for the first time, each patient will have need to complete Patient’s History Questionnaire. Therefore, to save patient’s time, it is good idea to download, print and fill the following attached Patient’s History Questionnaire prior to the patient visit to the sleep center.
